Comprehensive Geriatric Assessments

The Geriatric Assessment is often recommended for the older adult with unique health concerns associated with aging, particularly those with multiple chronic illnesses. With this thorough evaluation, our team assesses the medical, emotional, cognitive, and social needs of the older adult. Based on the assessment, recommendations are made to the patient and family.

A licensed, clinical social worker also provides gerontological counseling and support to the patient and family members. Because our social worker is intrinsically involved in the community, she's able to identify and secure community resources such as transportation services, Medicaid, and drug-payment options.

Following the initial assessment, our team will schedule a follow-up appointment and communicate results to the patient's primary care physician (PCP). For patients who do not have an established PCP, we will refer patients to one of our highly trained physicians.
